The Hospital Services Scheme
for Help and Support After Leaving Hospital

The Hospital Services Scheme provides support to older people, who live alone, in what can be difficult weeks after being discharged from hospital. We can ensure they have shopping on their return home or organise for a volunteer to stay overnight for up to 3 nights if they are anxious about being on their own. Volunteers can continue to visit for several weeks giving time and companionship, help with shopping and going out for a walk. They can also help by giving information and discussing individual problems. We can also help by accompanying older people home, who live alone and have had day procedure needing anaesthetic. We can ensure they arrive home all right, make them a drink and settle them in, if necessary be in contact the following day to ensure they are still OK.
